Memorandum for Property Insurance Carrier 
It's a letterhead from the Florissant Fire Protection District with contact info, a memo subject, and a note about a fire mitigation assessment.

Once wildfire assessment is complete you will receive the risk assessment worksheet as well as an official memorandum to give your insurance company
